    Thank you for trusting Deaconess Illinois Clinic with your care. We look forward to providing you with high-quality medical services and doing so in a warm, welcoming environment. 

    New Patient

    If you are a new patient, please bring your insurance card and arrive early to complete any necessary paperwork. If you have been under the care of another physician, please bring your records or have them delivered prior to your appointment.

    Accepted Insurance
    We accept most major insurance plans including Medicare and Medicaid. Please bring your insurance card with you so that we can make a copy for your medical record. If you have a change in insurance, address or phone number, be sure to notify the receptionist so that the change can be entered into our system.

    We accept all major credit cards for payment, such as Visa and Mastercard, and American Express. All balances are expected to be paid at the time of service

    Please call the office prior to your appointment to cancel. Twenty-four hour notice is greatly appreciated. If you don’t show up for your appointment, a fee may be applied, for which you are responsible since insurance will not cover this fee.

    Late Policy
    We are committed to prompt service and will work very hard, barring emergencies, to stay on time. We may ask you to reschedule if you arrive for your appointment more than 10 minutes after your scheduled appointment time. Please try to arrive 10 to 15 minutes early for your appointment to complete any paperwork associated with your visit.

    Prescription Renewals
    Please provide our office with at least 48 hours’ notice for all prescription refills. If you are asked to leave a message, always leave your full name, date of birth, name of the medicine(s) and dosage, as well as a contact number to return your call.
